We, for the membership fee paid in hand, do hereby apply for membership and participation in N.A.I.C. Tribal Org., an indigenous/ native American/ private religious/ Christian Church/ expressive association/ education and health care organization. With signing the membership agreement for any N.A.I.C. Tribal Org. Level of Authorized Membership Agreement I/we accept the offer made to become a member of N.A.I.C. Tribal Org. and have read and agree with the following Declaration of Purpose from Article I of N.A.I.C. Tribal Org—Articles of Association.

ARTICLE 1. This N.A.I.C. Tribal Org. Religious/ Christian Church Ecclesiastical, Religious Organization of members hereby declares that our main objective is to establish and protect our rights to religious/ ecclesiastical expressive association and freedom of choice regarding our indigenous and traditional. Familial, Native American, and Christian beliefs, education, and Holistic health information and care through maintaining our spirit have given us Constitutional rights.

ARTICLE 2. As members, we affirm our belief that the Constitution, Bill of Rights, and Amendments of the United States are one of the best documents ever devised by man, and the signers of the Declaration of Independence did so out of love for their country. We believe that the First Amendment of the Constitution of the United States of America guarantees our members the rights of free speech, petition, assembly, and the right to gather together for the lawful purpose of advising and helping one another in asserting our rights under the Federal and State Constitutions and Statutes. We strive to maintain and improve the civil rights, constitutional guarantees, and freedom of choice in religious expression, beliefs and practice, health care, and political freedom of every member and citizen of the United States of America.

ARTICLE 3. We declare the fundamental right of all of our members to select ministers, clergy, healers, pastors, counselors, therapists, physicians, medicine men and women practitioners, teachers, and medicine elders from our number who could be expected to give the wisest counsel, advice, and religious therapeutics concerning the need for Holistic physical, mental and spiritual health care assistance and to select from our membership those members who are the most skilled to assist and facilitate the actual performance and delivery of Christian Doctrine of Salvation and ministry, Indigenous Religious Therapeutics and Traditional, Native American Health Care, Ceremony, Native American Medicine and Medicine teachings, and Sacrament. We reserve the traditional right to practice Native and Indigenous, Traditional, Familial, and or Nature Cure healing and wellness supportive methods from any and all other indigenous/ native cultures concurrent with our own sacred beliefs such as, but not limited to:

Any Ministry Authorized for Ordained Christian Ministers, Lay Ministers, and Healers as defined in Holy Scripture The Bible: The five-fold ministry is found in Ephesians 4:11, which refers to the five roles God has called Christians to fill. These roles are (1) apostles, (2) prophets, (3) evangelists, (4) pastors, and (5) teachers. Each role is important for the early church and the body of Christ today to function properly.

The N.A.I.C. ministry will be a unique ministry school that will incorporate Biblical topics common to our respective Native American Indigenous communities and training that is mostly ignored by today’s mega-church mentality and deceived theologians. Our scope of the study will cover apologetics, doctrine, church history, spiritual warfare, eschatology, leadership, religious therapeutic, ceremony and healing ritual, and much more.

Any natural substance, water, plant or mineral, or energy (Seen or unseen) derived from Spirit and or the Sacred Mother Earth, Chirothesia, Holy Spirit, Holistic Health Care or Holistic Medicine, Ayurveda, Homeopathy, Herbology, Yoga Therapy, Hawaiian Huna and or Lomi Lomi, Filipino Hilot/ Ablon,  SomaVeda®, Thai Yoga, (Indigenous, Traditional Thai Massage slang), Nuad Boran, Jibarro- Taino Borekin Traditional Medicine of Indigenous Puerto Rico, care sessions, programs and or related, and suggest products, dense nutrition/ supplements, sacred food, diet, non-invasive electric instruments, and subtle energy devices using iontophoretic/ galvanic/ scalar/ radionic, homeopathic energy principles, as well bio-magnetic/ bio-energetic, Pranic, psychological, emotional, mental, Energy Psychology, or other physical/ energetic/spirit religious therapeutic based or originated healing methods concurrent with our sincere beliefs approved by N.A.I.C. church/ Tribal Org. N.A.I.C. Tribal Org. healing methodologies and understandings include developing therapies and non-standard approaches outside the scope of practice and or definitions of secular licensed medicine, including all secular allied medical specialties.

ARTICLE 4. We declare our Indigenous Ministerial/ Pastoral/Christian Clerical Religious Therapeutic heritage of traditional, natural Indigenous (clerical/ pastoral/ ministerial) Holistic Healthcare-Healing Science, Counseling, and Medicine (I.H.C.M.) as a continuing and vital heritage. I.H.C.M. is developed as a ministerial approach to health that incorporates indigenous, tribal, traditional, and scripturally valid science with Bible-based natural law to assess, improve, and restore good physical, mental, and spiritual health. The focus of I.H.C.M. is not on disease and treatment of disease but on analyzing the genesis of poor health and utilizing the natural law, including physical, mental, and spiritual insights for health from scripture and traditions derived from indigenous clerical medicine to bring about healing. I.H.C.M. does not intend to focus on disease or treatment of disease and avoids any techniques and substances that interfere with the natural order.

We believe that most disease has spiritual roots and see our role as supporting a vital, functional life as nature (spirit) intended. I.H.C.M. (Indigenous (clerical/ pastoral/ ministerial) Holistic Healthcare-Healing Science, Counseling, and Medicine) includes but is not limited by the following:  Ministerial/ Clerical/ Pastoral/ Medicine person or Elder practiced Therapeutics, Ministerial/ Clerical/ Pastoral/ Medicine person or Elder practiced Counseling, Ministerial/ Clerical/ Pastoral/ Medicine person or Elder practiced Clinical Natural Medicine or Ministerial/ Clerical/ Pastoral/ Medicine person or Elder practiced Traditional Christian Monastic-Naturopathy and or Traditional Christian Monastic Nature Cure. These programs establish a basis for professional I.H.C.M. medical standards and licensing (Licentiate-Tribal Authorization), supporting the fullest scope of practice in spiritually based therapeutics for mind, body, and spirit.

Evangelism: We are empowered to Share God’s Word (this can be salvation, healing, deliverance, biblical cosmology, etc.) with both believers and non-believers.

N.A.I.C. Tribal Org. Spiritually based healthcare (Christian/ Monastic/ Bible-based) therapeutics include and are not limited to Religious Therapeutics: Medicine of Hope: Sacred Nutritional Counseling, Clerical Physical and or Manual Therapy (Chirothesia), Psychological/ Emotional Therapy, Counseling for Mental Well Being, Healing, Casting out of Demons, the use of indigenous, traditional Native American and Indigenous ritual/ ceremony and Sacrament for total well being (N.A.I.C.), Non-invasive standardized evaluation and assessment, Clia-Waived testing and assessment protocols, Multi-modality therapeutic approaches using any natural energies, ceremonies/ rituals/ meditations/ prayers/ Puja, sacraments or substances that spirit and nature have provided. Our healing Sacrament and apothecary or “Medicine Chest,” is intended to heal those of the sick and the infirm using natural agents and products such as foods, soups, herbs, teas, salts, oils, fats, and water; specific cures and tried traditional remedies; and supernatural agents including spiritual counseling, prayer, divination, worship, fasting, application of sacred waters, sacred salts, sacred foods; herbs, oils, and perfumes. We aim to restore the harmony of nature, man, and spirit to oneness.

These practices DO NOT include the potentially dangerous use of invasive techniques and toxic drugs that are restricted or access controlled by Federal or State statutes and or the province of licensed medicine.

Important, Please Note: Although we believe in the rights of Native Americans/ Native American Church/ Native American Indigenous Church/ Native American Tribal Organizations to use particular psychotropic entheogenic sacraments, when used specifically for their psychotropic effects, which are at this time federally regulated (DEA), such as Peyote, San Pedro, Ayahuasca, Cannabis, and Cannabis derivatives, Kambo, Ibogain, Sacred Mushrooms, or any similar substances, whether listed here or not:

Although the everyday use in religious rights and traditional medicine practices of many different Native and or Indigenous Tribal societies is well established, these sacraments are still held in controversy and subject to erratic policy, interpretation, and enforcement from jurisdiction to jurisdiction. Therefore, until the matter is settled and all of our members can comply with both traditional rules and modern interpretations of the relevant statute, we, N.A.I.C. Tribal Org., Independent and free church organization and our subsidiaries (and or integrated auxiliaries), do not possess, use or distribute any federally regulated substances of any kind whosoever, unless located in a state where the right has been clearly and legally established and or authorized under existing DEA compliance rules where the right has been clearly and legally established. Unless the use and ceremony are entirely conducted by an N.A.I.C.-recognized medicine person, with active N.A.I.C. Enrolled members, on or off a registered tribal land, reservation, or recognized tribal sanctuary.  Suppose there is any question as to whether a medicine person is Authorised to use or distribute any specific sacraments. In that case, we strongly urge you to contact the N.A.I.C. directly and make a query. Nor do we use any controlled substances in any of our N.A.I.C. Tribal Org. ceremony, practice, or medicine unless approved in the State of residence and or by DEA by exemption process. Nor do we promote or allow non-members or the public to participate in N.A.I.C. Tribal Org. Ceremonies. All N.A.I.C. Tribal Org. Ceremonies are for N.A.I.C . Tribal Org. members only and NOT for the public. Participants must have a signed N.A.I.C. Member form and Waiver on file to receive or participate in any services. There are 10’s thousands of sacred herbs and substances that are not controversial nor subject to any governmental authority that we can safely and wisely use.

ARTICLE 5. We proclaim the freedom to choose and perform, for ourselves, the types of sacred healthcare, guidance, counseling/ healing, and well-being-making modalities that we think are best for achieving and maintaining optimum wellness equilibrium, internal and external,  of our minds, emotions, spirits, and bodies. We proclaim and reserve the right to include health maintenance and support options that have but are not limited to cutting-edge modalities practiced or used by any types of indigenous ministers and healers, counselors, therapists, or practitioners the world over, whether traditional or nontraditional, conventional or unconventional. We believe, and it is our sincere and firmly held conviction, that it is a Human Right given to us by the creator God (Great Spirit- “Creator Sets Free“) to have access to and to be able to practice healing for one’s self, one’s family, and the community of fellow human beings. We affirm this right under US and International conventions, such as in the Universal Declaration of Human Rights (UDHR) declaration of the United Nations General Assembly (10 December 1948, Palais de Chailot, Paris) and the Nuremberg Code.

IMPORTANT NOTE: N.A.I.C. is a Humanitarian Aid Mission and Society offering Humanitarian Aid and Services.

ARTICLE 6. More specifically, the mission of our N.A.I.C. Tribal Org. N.A.I.C. Tribal Org. Church Member Association is to provide members with the highest quality Humanitarian Aid Care and Services by the most effective care methods. N.A.I.C. Tribal Org. understands that wellness has many dimensions and strives daily to stay on the leading edge of new products and technology. The church provides advanced products and technologies for assessing a member’s health and provides the most effective care at an affordable fee. More specifically, the church and church members specialize in recommending, counseling, and selling food and food-based supplements and other products on the wholesale and retail markets and offer electronic instruments to these markets as alternates to medication concerning the modalities of service and benefits to members. Authorized therapists, counselors, practitioners, providers, or medicine persons do not diagnose or prescribe for medical or psychological conditions nor claim to prevent, treat, mitigate, or cure such conditions.

ARTICLE 7.  N.A.I.C. Tribal Org. will recognize any person (irrespective of race, color, sexual orientation, or religion) who is following these principles and policies as a member and will provide a medium through which its individual N.A.I.C. Tribal Org. members may associate for actuating and bringing to fruition the purposes heretofore declared. NAIC does not respect or observe restrictions to access based on “blood quotient.”

ARTICLE 8. Vaccination/ Vaccine Resolution: The Native American Indigenous Church INC (N.A.I.C. Tribal Org.)  finds that inquiry from residents and visitors to the N.A.I.C. Tribal Org. concerning immunization necessitates an immediate resolution from this Native American Church ecclesiastical jurisdiction, establishing our position in this matter. In making this critical decision, we shall address the following frequently asked questions;
• Does the N.A.I.C. Tribal Org. believe that vaccines are responsible for neurological disorders such as Autism?
• As an Indigenous Tribal and Native American Church jurisdiction, does the N.A.I.C. Tribal Org. support immunizations?

Regarding whether we believe immunizations cause neurological disorders, we believe that the responsibility of N.A.I.C. Tribal Org. members and participants/ believers is to judge a thing first on its sacred, traditional, and or scriptural validity, not on its scientific basis. We do not find any holy, indigenous, traditional, or scriptural support for immunizations. It can be argued that multiple teachings and traditions verses speak not only against defiling the body, as the body is considered to be sacred and “soil,” but provide spiritual and hygienic instruction that, if followed, would negate the need for such immunization.

A close and honest review of the historical data surrounding immunization leads a reasonable individual to conclude that the immunity of populations derived primarily from increased awareness of hygiene, the integration of clean, healthy living in harmony with the land, and not from vaccination (further validation of our traditional teachings and scriptural instruction). From a Pastoral Health Care and Holistic Health Care and Medicine position, the same holds; the base philosophy of the creator’s natural health care is not to design immunity against man-made toxins but to remove and avoid those toxins that impede the natural flow of immunity imparted to mankind by the creator spirit and Mother Earth. It, therefore, follows that the N.A.I.C. Tribal Org. has no further responsibility or need to debate the efficacy of immunization. To do so would be akin to doubting the word and wisdom of our ancient traditions and customs brought forward to the present day. Such unbelief and questioning of the creator’s wisdom and instruction set a man on the path to separation from all that is holy, sacred, and of integrity with oneness with all our relations.

N.A.I.C. Religious Injunction: Thus says the Source: “Thou shalt not allow the entry of abominations and unholiness into the bodies of thine children and thine own selves, for they are Gods’ Temples and sacred unto thine Source.”

1 Corinthians 6:19 ESV:  “Or do you not know that your body is a temple of the Holy Spirit within you, whom you have from God? You are not your own.”

N.A.I.C. communicants are directed explicitly by their ecclesiastical supervisors to object to and refuse any of the following unholiness and abominations, entering the Body Temples of themselves or their children as a violation of the free exercise of their religious beliefs and the moral obligations of their religion: the administering of vaccinations, human or other blood, chemotherapy, radiation, and other non-natural remedies are declared to be unholy abominations; the use of injection or otherwise to administer abominations is declared to interfere with the practice of religion.

The making of this Religious Injunction and adhering to it by communicants is declared the Religious Expression of the Native American Indigenous Church and an exercise of our Expressive Association.

N.A.I.C. Tribal Org. must and does, therefore, hold the sincere religious belief that vaccines in any form are abhorrent to nature, spirit, and the creator, counter to sacred teachings and beliefs passed on through our oral and ceremonial traditions as well as scripture (Christian Bible) and should be refused by believers. The authority and privilege of this N.A.I.C. Tribal Org. ecclesiastical jurisdiction to refuse vaccination extends to all members and Communicants who have expressed their choice to refuse vaccination for their families by having completed and filed the required form with the N.A.I.C. Tribal Org. office. Additionally, we recognize the value of non-invasive Homeopathic vaccinations as a substitute for any contaminated, adulterated, or dangerous conventional immunization. Religious Injunction: Obligations of Communicants and Instructions to Parents

Based upon the consensus of the ministers and elders, this Statement of Doctrine and Religious Injunction is posted on the Church website, NativeFireChurch.org, as a Church By-Law for downloading by all believers and communicants who are hereby authorized to present same to school or other authorities as the basis for our religious objection to any vaccination or other abominations that violates our Belief in the sacred nature of the Person, by including any of the spiritually objectionable substances listed above, or by being administered in violation of the Religious Injunction set forth herein.

Parents are urged to object to vaccination upon the ground that the proposed immunization interferes with the free exercise of the pupil’s religious rights and that the administration of vaccination or other abomination interferes with religious belief and exercise, violating the Expressive Association of the communicants.

ARTICLE 9. In furtherance of N.A.I.C. Inc Tribal Org. Educational Mission We establish the American College of Natural Medicine (A.C.N.M./S.C.N.M.),  The Thai Yoga Center, and LearnThaiYoga™ On-Line as extensions and seminaries of N.A.I.C. to provide quality education in the spiritual and traditional principles, practices, therapies, counseling competencies necessary to administer our work and mission for a suffering world. A.C.N.M./S.C.N.M. will administer all Certificate, Diploma, and Degree programs and is authorized to issue all such certificates, diplomas, and Degrees, including but not limited to Associates in Sacred Arts, Bachelor of Sacred Science, Bachelor of Applied Sacred Science Massage TherapyBachelor of Applied Sacred Science Yoga Therapy, Doctor of Sacred Natural Medicine (D.S.N.M.), Doctor of Sacred Arts in Traditional and Indigenous Medicine (D.S.T.I.M.) and Ph.D. in Traditional Ayurveda Medicine (Ph.D.A.M.). We also empower A.C.N.M./S.C.N.M. to make any educational affiliations with other like-minded Tribal bodies, organizations, associations, churches, schools, and institutions as may support the breadth and competency of our academic programs. N.A.I.C., A.C.N.M./S.C.N.M./ Thai Yoga Center program graduates will be empowered by their education and duly recognized by N.A.I.C. with awards and confirmations of registry, commission, authority, ordination, and Licentiate to practice Tribal Health Care as deemed appropriate by the church. N.A.I.C. Tribal Org. Ordained Ministers and Independent Branch Elders must complete a course of study and seminary to educate and prepare them for their duties and responsibilities as N.A.I.C. Tribal Org—Ordained Ministers representing Indigenous and Traditional N.A.I.C. teachings and ceremony.

ARTICLE 10. Edification: We believe the Home Cell is the basic building block of the church. It is, in fact, a mini-church. In a home cell, edification occurs as N.A.I.C. Tribal Org. members care for one another, using spiritual gifts and support systems.  A Home cell is a group of two or more N.A.I.C. Tribal Org. Members form the basis of an active spiritual community, where members gather around in the presence of Spirit, Mother Earth, and God, support one another as a family, reach out in service, share and exchange healing, ceremony, religious services, Sacrament, and sacred duties both for themselves and for the suffering world as well as mentor, educate and support new members, healers, practitioner/ providers, teachers, and leaders. The ultimate goal of the Home Cell is to multiply itself as the tribal organization expands and grows from generation to generation. We define the edification process: To honor the spirit within us, to build that spirit within and without, to serve the spirit within each sacred person, and to bring healing and medicine to all as our legacy, cultural heritage, and imperative. To learn the way of holy nature and accept the wisdom of nature communicated to us directly in ceremony, ritual, and the sacred practices we have been given.

ARTICLE 11. Clergy/ Penitent Privilege (Ecclesiastical Physician/ Medicine Person/ Patient): Under church doctrine, a confession includes an expectation of confidentiality. We expected confessions to remain confidential as a matter of church doctrine, as evidenced by our established doctrine, member agreements, and waiver/consent forms. We emphasize the need to maintain confidentiality regarding issues shared by church cells. This confidentiality is essential in furthering our faith and exposing, exploring, and mediating the root causes of all illnesses and conditions, which we believe to be factored in generating and maintaining a state of optimal health and wellness of the Spirit, mind, and body.

ARTICLE 12. Agree to N.A.I.C. Church Arbitration exclusively: Clergy-Minister/ Penitent Privilege (Physician, Therapist, Practitioner, Medicine Person/ Patient) is Extended to Church Arbitration: We Agree that the N.A.I.C. Tribunal (Church Arbitration by Board of Elders: B.O.E.) shall be the exclusive jurisdiction and remedy for resolving any conflicts or complaints of any kind- in whatever form – monetarily or otherwise affecting the N.A.I.C. – between the Parties or any N.A.I.C. participants, members, ministers, elders, students, N.A.I.C. members and or third parties outside of the curch-tribal organization such as State Boards of Medicine, Allied Medical Boards, Massage Boards, Health Boards or Law Enforcement of any jurisdiction whether local, state or federal.  Participant/ Communicant further acknowledges that in the event they are dissatisfied with any service provided by a fellow member, Authorized Minister, commissioned and or Licentiate, if and in the event the Tribunal finds on behalf of the complainant, the maximum remedy the ecclesiastical process may award in such case is entirely at the discretion of the examining board of N.A.I.C. elders appointed and or elected to hear the issue. In all cases, contents, deliberations, outcomes, member names, or identifying information of either board of examiner members or participants will be private as church-protected communication. All participants will agree to this as a precondition to any hearing or deliberation. Member participant agrees in advance to a penalty of not less than $100,000.00 due on notice of violation. A breach of this Arbitration-Non-Disclosure Agreement shall constitute a breach of N.A.I.C. Member agreements.

MEMORANDUM OF UNDERSTANDING

We understand that the fellow members of the N.A.I.C. Tribal Org. that provide education and care do so in the capacity of a fellow member and not in the capacity of a secular or state-licensed health care provider. We further understand that within the church association, no doctor-patient relationship exists but only a private, ecclesiastical/ confessional/ communicant (Priest-penitent Privilege) privileged member-to-member religious organization (N.A.I.C. Inc. Tribal organization/ Faith Based Organization) and relationship.

In addition, We have freely chosen to change our legal status as a public person or patient to a private member of the N.A.I.C. Tribal Org., native branch indigenous peoples religious organization) to receive, practice, and exchange services with other members.

We further understand that it is entirely our responsibility to consider the advice and recommendations offered to us by my fellow N.A.I.C. Tribal Org. members and to educate ourselves as to the efficacy, risks, and desirability of same and the acceptance of the offered or recommended program, care and products is our own carefully considered decision. Any request by us to a fellow member to assist us or provide us with the care mentioned above is our own free decision in an exercise of our rights and made by us for our benefit. We agree to hold the Director(s), Ministers, and Healers. However, they are titled, staff and other worker members and the N.A.I.C. Tribal Org. harmless from any unintentional liability for the results of such care, except for harm that results from instances of a clear and present danger of substantive malicious intent as determined by the N.A.I.C. Tribal Org., as stated and defined by the United States Supreme Court. Members have the right to choose whomever within the N.A.I.C. Tribal Org. Health Care Member Association is best qualified for their wellness and healing needs and practices.

In addition, We understand that since the N.A.I.C. Tribal Org. is protected by the First and Fourteenth Amendments to the U.S. Constitution, it is outside the jurisdiction and authority of Federal and State Agencies and Authorities concerning all complaints or grievances against the N.A.I.C. Tribal Org., any Director(s), members or other staff persons. An N.A.I.C. Tribal Org will settle all rights of complaints or grievances. The member will waive the Committee of Elders/ Ministers for the benefit of N.A.I.C. Tribal Org. and its members. Privacy and security of church membership records maintained within the N.A.I.C. Tribal Org. and all religious organizations have been held to be inviolate by the U.S. Supreme Court. We agree that violating any waivers in this membership contract will result in a no-contest legal proceeding against me. In addition, the N.A.I.C. Tribal Org. does not participate in any medical insurance plans or collections on behalf of the member.

I/ We agree to join the N.A.I.C. Tribal Org., a private religious/ church, an expressive private association membership organization/ association under common law, a Tribal Organization, A Christian/ Christ-Centered organization whose members seek  to help each other achieve better health and live longer with good quality of life through principles of Indigenous, Traditional and Native American Medicine, Chirothesia, healing and wellness practices and traditions, as well as indigenous traditional medicine or therapies such as, but not limited to Native, Indigenous and or Tribal, Ayurveda, Lomi Lomi, Hilot/ Ablon, Taino/ Borenkin, SomaVeda®, Thai Yoga and/or Traditional Thai Massage as well as any other natural, non-invasive, Bible Based, Christian Monastic Medicine Based, Mother Earth Based, holistic art and science for healing approved for use within the N.A.I.C. Tribal Org. sacred ideas, medicine systems and way of life brought to us by our elders, divine inspiration and vision are elegant, practical and evolving solutions for suffering, pain and lack of connection which separate us from the knowledge of healing, true medicine and an excellent relationship to our Mother Earth and all living things.

Simply put, we claim our inherent legal and federally protected right of self-determination in expressing and practicing our indigenous religion and religious therapeutics. We claim the expressed right to believe, profess, practice, and express without undo constraint any or all indigenous, native, aboriginal, traditional medicines and or medical practices or therapies that we adopt as core medicine and healing practices of our tribal entity- organization.

We understand that the tribal organization Ministry and N.A.I.C. Humanitarian Aid Service Holistic Care Mission providers who are fellow members of the N.A.I.C. Tribal Org. offer I/ me health care, advice, services, and benefits that do not necessarily conform to conventional medical care or secular Western Allopathic standards of care. I do not expect these benefits to include on-call coverage, hospital care, or most physicians’ usual and customary care. We will receive such primary and specialist care elsewhere. We fully understand that the benefits we receive from N.A.I.C. Tribal Org. are probably not covered by my health insurance or Medicare.

As a member, I/ we accept the goals of helping my body function better and choosing techniques that are both very safe and have a reasonably good chance to succeed, realizing that no evaluation technique or remedy is foolproof. If we choose to forgo drugs, surgery, or radiation recommended to us by others, we fully accept the risk that we might suffer severe consequences. Other aspects of the disclosure and informed consent will occur in our private discussions with the providers and fellow members of the N.A.I.C. Tribal Org. Our activities within the N.A.I.C. Tribal Org. are a private religious matter and practice of participation that we refuse to share with the State Medical Board, the FDA, Medicare, Medicaid, or my insurance company without our expressed specific permission. All records and documents remain as property of the Association, even if we receive a copy of them. We fully agree not to file a malpractice lawsuit against a fellow member of the N.A.I.C. Inc. Tribal Church unless that member has exposed us to a clear and present danger of substantive evil. We acknowledge that the members of the N.A.I.C. Tribal Org. are not required to and do not carry malpractice insurance.

We enter into this agreement of our own free will or on behalf of my dependent without any pressure or promise of a cure. We affirm that we do not represent any state or federal agency whose purpose is to regulate the practice of medicine. We have read and understood this document, and my questions have been satisfactorily answered. We understand that we can withdraw from this agreement and terminate our membership in this organization at any time. These pages, Individual Membership Agreements, Article I of the Articles of Association of the Association, Mission StatementThe Code of Ethics, and specific membership rules for each level being applied for, consist of the entire agreement for our membership in the Tribal Org. and Health Care Association and they supersede any previous agreement.

We understand that the membership fee entitles me to receive those benefits declared by the association director(s) to be “general benefits” free of further charge. We agree to pay all fees for services, exchanges, or courses as agreed upon ourselves and any Student/ Practitioner/Therapist/Counselor/Teacher/Minister/Licentiate level members we choose to make agreements with. We have paid membership donations appropriate to our level of membership as consideration for our membership contract, said term beginning with the date of the signing of our membership agreement, subject to annual renewal, and by these presents, do now certify, attest, and warrant that we have carefully read the above and preceding N.A.I.C. Tribal Org. Articles for the Organization, and fully understand and agree with same.
